It is primarily caused by mutations in the 11?-hydroxysteroid dehydrogenase type 2 (11?-HSD2) enzyme, which normally protects the mineralocorticoid receptor from activation by cortisol. In individuals with AME, increased cortisol levels can bind to these receptors, mimicking the effects of aldosterone, resulting in sodium retention, potassium loss, and subsequent high blood pressure. Early diagnosis and management are essential to prevent complications associated with long-term hypertension.
